How do I avail of your Test Services?

Step 1

Present any valid ID to the Customer Service Counter (CSC) and register at the Visitor’s Log-in System. The CSC staff will issue you a “Visitor’s ID” card.

Step 2

The CSC Staff will assist you to the Sample Receiving Room. Assigned technical staff will attend to you shortly.

Step 3

If a request can be accommodated, fill out the Technical Service Request (TSR) form and submit to the attending technical staff for completion and approval. Approved TSR will indicate actual test fees, due date, and TSR number.

Step 4

Submit cash/check payment to the CSC Staff. Wait for the payment to be processed. The CSC Staff will give you the Official Receipt (OR) and a copy of the TSR form.

Step 5

After transaction, surrender the “Visitor’s ID” card to the CSC Staff. The CSC will then return your ID.

How do I claim my report?

Step 1

Present any valid ID to the Customer Service Counter (CSC) and register at the Visitor’s Log-in System. The CSC staff will issue you a “Visitor’s ID” card.

Step 2

Present a copy of the Technical Service Request (TSR) form or Official Receipt (OR) to the CSC Staff for verification.

Step 3

The concerned technical staff will release the Report of Analysis (ROA). Sign the receiving copy of the ROA and the Exit Pass for Customer Property form to claim the remaining sample. Fill out the Customer Satisfaction Survey Form then submit it to the CSC Staff.

Step 4

After transaction, surrender the “Visitor’s ID” card to the CSC Staff. The CSC will then return your ID.
